Do I Need A Breast Lift Or Just An Augmentation? Ask Dr. Brian Evans

If you want to get some enhancement done on your breasts, it is fortunate today that there are a number of options available for you. And the best part is that these procedures can cater to your specific needs. But how do you know when you are going to need a lift or a breast augmentation surgery?

Dr Brian Evans often faced with that question from patients that feel their breasts are saggy. Sometimes the answer is obvious, but some other times it’s not that clear cut. Most of the time patients that would benefit from a breast lift already know the answer and they come in requesting that procedure. But there is a selected group that doesn’t want to do a lift but are afraid they are going to need it.  Let’s digress here for a moment; nobody really needs a lift, like nobody needs a diamond. You get it because you want it, it’s not essential for life but it makes you happy to have it. Similarly, getting a cosmetic surgical procedure may improve your quality of life, as you are no longer self-conscious about a certain part of your anatomy.

Having said that, the purpose of a lift is to improve the shape and form of a breast that has otherwise experienced profound changes due to pregnancy, breast feeding, loss or gain of significant weight or sometimes they simply developed with an unattractive shape. Most people can tell a pretty breast when they see it and that is because an attractive breast has defined proportions. They are nice and round, they are not too long, and they have nipples and areolas that are located centrally and facing forward.  The skin is firm and so is the glandular tissue and even though these last two items cannot be changed we can surely change the proportions of the breast with a lift. Most lifts can be done simultaneously with an augmentation to make the breasts larger but in extreme cases it’s better to do them separately or in stages.

So, how do I know when the breast will need a lift?  The answer is in the proportions. Dr Brian Evans MD measures the distance from the collarbone to the nipple and if this is greater than 24 cm then most likely it will need to be lifted. Another clue is orientation of the nipples; if they are pointing straight down then they need to be lifted regardless of the measurement. The size of the areolas (the dark skin that surrounds the nipple) is also important, as they get larger when the skin is saggy or stretchy.  Reducing their size along with centralizing their position is an essential part of restoring ideal proportions of an attractive breast. This is commonly referred by the vernacular as to going from “bologna to pepperoni”. The ideal size of an areola is about 4 to 6 cm in diameter but certain allowances can be given based on personal preference.

So far we’ve learned that a breast lift, also called a “mastopexy”, is performed to restore attractive proportions when the original breast has developed unfavorable ones. More often than not a mastopexy is performed in conjunction with an implant augmentation to improve the volume of the breast as well as enhancing the roundness and fullness of it. When an augmentation is performed alone in a breast with unfavorable proportions the results can be disastrous. There will be a mismatch between the breast and the implant that will appear worse than the original problem. Some practitioners will try to get around that problem using larger implants in order to expand and fill the saggy stretchy breast. Most often that will lead to unreasonable breast sizes that invariably make the patient uncomfortable and eventually the breast will sag even more in the short term. Fortunately, this situation can be corrected by providing the lift in conjunction with a reduction in the size of the implants. The conversion to silicone implants under the muscle along with a mastopexy has proven to be a very useful procedure in my practice restoring breast harmony in many grateful patients.

The reason many patients are reluctant to consider a lift is because in order to modify the unfavorable features of a breast you need to create skin incisions and therefore scars that are permanent. Luckily, new techniques available for this minimize the amount of scars. Silicone Gel and Breast Implants - Dr. Brian Evans

About Silicone Breast Implants 

On November 17, 2006, the FDA approved silicone gel implants. Now that the products have been determined to be safe and effective, the FDA will continue to monitor them by requiring each breast implant manufacturer (Mentor and Inamed) to conduct a large post approval study following which will follow about 40,000 women for 10 years after receiving breast implants. The FDA often requires post market studies to answer important questions that can only be answered once a product is in broader use, such as the incidence of rare adverse events.


The FDA's decision to approve these implants was based on a thorough review of each company's clinical (core) and preclinical studies, a review of studies by independent scientific bodies and deliberations of advisory panels of outside experts that heard public comment from hundreds of stakeholders. In addition, the FDA conducted inspections of each company’s manufacturing facilities to determine that they comply with the FDA Good Manufacturing Practices. Some of the complications reported in the core studies included hardening of the area around the implant, breast pain, change in nipple sensation, implant rupture and the need for additional surgery. However, the majority of women in these studies reported being satisfied with their implants.

In the past decade, a number of independent studies have examined whether silicone gel-filled breast implants are associated with connective tissue disease or cancer. The studies, including a report by the Institute of Medicine, have concluded there is no convincing evidence that breast implants are associated with either of these diseases. However, these issues will be addressed further in the post approval studies conducted by the companies.


About Silicone Gel

Silicon is the second most abundant element on earth, next to oxygen. Silicones are actually a family of chemical compounds. They are made of silicon, which is a naturally occurring element found in sand, quartz, and rock. When silicon is mixed with oxygen, hydrogen, and carbon, it becomes silicone. Silicone can be made in a variety of forms. Low molecular weight silicones form oils. Middle molecular weight forms gels, and high molecular weight forms elastomers and rubbers. Silicone is found in many other types of implants, such as facial implants, artificial joints, drainage systems, etc.

Currently, Mentor and Inamed silicone implants have 3 layers of shell surrounding the silicone gel. This reduces the amount of gel bleed. It does, however, make the implants a bit firmer than they were years ago, but this is attributed to the thicker shell. Silicone implants of today are still very soft and natural feeling.

Silicone gel breast implants come pre-filled, meaning that there is no fill to be added. The only real drawback to a pre-filled implant is that the incision will have to be a bit longer, depending on the size of the implant to be inserted. This is especially true for textured silicone and gummy bear silicone implants, which are somewhat firmer. Another reason that textured silicone implants require a longer incision is the fact that the shell is textured. Because silicone gel breast implants come pre-filled, they cannot be inserted via the TUBA incision (nor can any other type of pre-filled implant).

Buttock augmentation with your own fat is yet another approach to add fullness, projection and shape to the under projected or flat buttock area. Also known as the Brazilian butt lift, this procedure involves a selective "gentle" liposuction technique, harvesting fat from those areas around the buttocks that tend to hide a more desirable shape and then transferring the fat into the proper areas of the buttocks in multiple layers. Harvesting the excess fat from the sides, above, below, and around the buttocks first enhances the curves and projection prior to the micro fat transfer and makes for an even shapelier derriere. Liposculpture and fat harvesting for buttock augmentation can use excess fat from numerous other areas including the abdomen, back, hips, waist, thighs, knees, and arms as needed

As the demand for more shapely bottoms skyrockets, these same techniques have been adapted to augment and shape the buttocks for those who are good candidates. The key to success with any fat augmentation procedure is micro fat grafting or transfer.  Fat is harvested using a low-pressure gentle liposuction method, processed and transferred into small syringes connected to micro canulas of 1.5 to 2 mm in diameter.
Each syringe of fat requires multiple passes, placing thousands of bead like droplets of fat cells into multiple layers of tissues and in multiple directions through these micro canulas.  These small bead-like droplets of fat can establish a new blood supply more easily than a larger mass of fat.  Micro fat transfer starts within deep layers of the muscle, in an interlacing fashion, and works slowly toward the superficial fat layers just under the skin.

Layering these small droplets of fat essentially one by one, allows for the best blood circulation to the fat cells and maximizes fat cell survival producing smooth, rounded contour and the best results. For example, a transfer having micro fat augmentation of the buttocks of 500 cc for each side will require nearly three thousand passes of the micro canulae

This takes time and patience with an average procedure requiring 4-6 hours. Not all of the fat will survive, some will be reabsorbed. No surgeon can accurately predict exactly how much fat will survive; however, it is believed that what remains after three months will remain permanently.

Micro fat augmentation of the buttocks will not result in lumps and bumps and will not cause cellulite. Actually fat has been used for years to correct irregularities and deformities of various parts of the body including the buttocks. Adding fat volume to the buttocks will look and feel completely natural as well as improve contour as is it has done in the face by experienced surgeons for years.
